School Attendance Letter Request

If you are required to prove your child attends Dagenham Parch Church of England school, you will need to follow this procedure.

You are required to submit this form to office@dagenhampark.org.uk, and you must provide 2 documents. One document to prove your address. The other for us to see where the letter needs to be addressed to and why the letter is needed.

You do not need to call the school. If you have given all the correct information your letter will be processed in 48 hours.
